After a successful bid to the Big Lottery's Investing In Communities Fund, SHARE Scotland are pleased to announce their latest project.

The West Dunbartonshire Transtion Project will be run in partnership with Cornerstone, and will provide support and guidance to young people aged 13-19 with additional support needs (learning disability, autistic spectrum disorder, ADHD) and their families in West Dunbartonshire.  The project will run until 2012, and we hope to work with 90 families over the course of the project.

We will help you gain the knowledge, confidence and skills needed to move into employment, education, independent living, and to be more involved in the community.

Everyone who uses this service will receive:

* One to one help from a dedicated Transitions Worker, who will be committed to supporting you for the duration of the project.
* Independent support, information and advice, individually tailored to each family's needs.
* A Personal Life Plan, adapted to any communication needs, which will grow and change with you as much as your own ambitions and goals.
* Access to a Buddy volunteer, a young person from a local school who wants to get you involved in community projects in your area.
